Are you passionate about helping vulnerable individuals break free from the cycle of rough sleeping?

A leading homelessness charity in London is seeking a compassionate Case Worker to work closely with a cohort of 100 clients with extensive rough sleeping histories. Your role will involve developing innovative solutions, fostering strong support networks, and guiding these individuals towards sustained and safe accommodation, empowering them to leave the streets behind.

Key Responsibilities:

• Deliver impactful, creative, and holistic services that drive tangible results
• Implement a Personalisation and Recovery methodology to empower clients
• Collaborate with partners, peers, and agencies to create joint recovery plans
• Engage clients using motivational techniques and empathy to facilitate their transition from street life
• Provide comprehensive care across areas like work, health, housing, and relationships

Engagement, Assessment, and Case Management:

• Engage with clients in diverse settings, accommodating flexible hours
• Use case management systems to guide interventions based on recovery plans
• Develop and deliver multi-agency recovery plans, challenging detrimental practices
• Build links with employment, training agencies, and specialists to cater to diverse needs
• Establish personalised recovery networks, incorporating digital inclusion, peer support, and volunteer involvement

Essential Requirements:

• Proven success in delivering positive outcomes for vulnerable and hard-to-reach groups
• Proficiency in employing psychological techniques to work with vulnerable adults
• Effective communication, assessment, and client-centered planning skills
• Understanding of complex needs in health, housing, and support for rough sleepers
