Mental Health Support Worker

About our clients' Service: Join the dedicated team working in the Waltham Forest area, where we operate four supported houses, a small floating support service, and a day program for adults experiencing mental health challenges. The primary goal is to empower individuals with ongoing mental health issues to thrive in the community, maintain their mental well-being, and progress toward independent living.

Position Details:

  • Job Type: Full-time, 37.5 hours per week
  • Work Schedule: Monday to Sunday on a rotating basis
  • Shifts: Variable shifts (10 hours, 8 hours, and 5 hours) between 9 AM - 8 PM
  • Coverage: Ongoing on a rolling monthly basis

Responsibilities: As a Project Worker, your role will include:

  • Managing a caseload of 12 key clients
  • Motivating and coaching clients to attend appointments and participate in activities
  • Maximising income for clients and the service through regular benefit reviews
  • Recording needs and risk assessments in the system
  • Completing referrals and attending move-on assessments as an advocate
  • Assisting clients with the process of moving into the scheme
  • Addressing practical accommodation-related issues, including risk assessment
  • Collecting information to inform assessment and support planning
  • Managing rent and service charge payments, attending statutory appointments, and ensuring compliance with treatment or criminal justice requirements
  • Supporting clients to engage in activities that promote independence and community involvement
  • Ensuring necessary information for referrals to other services is available and forwarded
  • Building clients' abilities and confidence through coaching and 1:1 support to enhance independent living skills
  • Establishing and maintaining strong connections with local statutory and voluntary agencies and community resources
  • Developing opportunities for clients with grant-giving charities or specialist agencies to facilitate move-on options and access primary and other healthcare services
