Pan London Rough Sleeper Navigator Service

Are you passionate about helping those in need and ending homelessness? We have 2 exciting positions available within our experienced Rough Sleeper Navigator team. Join us in our mission to support long-term rough sleepers on their journey from the streets to a stable home.

Here is what you will be doing:

• Operating a dedicated phone line to provide assistance for individuals at risk of returning to rough sleeping
• Conducting thorough assessments and delivering brief interventions to help them sustain their recovery and retain their accommodation
• Linking people to appropriate support within their community, ensuring they receive the help they need
• Helping them set up their new homes, connect with their community, and develop the essential skills for independent living
• Working alongside Navigators to lay the groundwork for individuals with significant rough sleeping histories to sustain their accommodation
• Providing ongoing support and guidance to empower them to maintain stable housing in the long run
