Lead Outreach Worker

Job description

I am currently looking for a Lead Outreach Worker to work for one of our charities based in East London. The role is Monday - Friday 7.00am - 3.00pm but due to the nature of the role you will be required to work the occasional evening. The role will be around a 6 months contract with the possibility of extension.

The main purpose of the role is to organise the street outreach shifts for the team from the referrals and mapping information you receive within the borough. Also, you will be required to deliver assessments to new rough sleepers and ensure that all case work is completed for new and existing clients.

Duties:

• Supporting rough sleepers and ensuring referrals are made to relevant services/accommodations
• Organising the rota depending on the needs of the referrals
• Ensure assessment and verification is provided to new rough sleepers to ensure appropriate support is identified
• Supporting clients to access accommodation, health services, day centres and housing advice services
