  • Resettlement Team Support Worker Needed!

    Location: South & West London boroughs

    Social Care Assignment: 2-3 Months

    Hours: Mon - Fri, 09:00-17:00

    Must have an enhanced DBS Issued within the last 12 months (or on the update service)

    Pay: £13.50 holiday pay = £15.13 or £17 Umbrella per hour

    Our social care client needs a Support Worker to join their resettlement team. In this role you will provide tenancy related support to vulnerable ex homeless clients who are moving into new accommodation. You will be supporting clients to ensure that they have everything they need when they initially move into the property. This will assist clients to ensure they maintain their tenancy further down the line and it will enable the Tenancy sustainment team to produce far better work, knowing that the client has been set up correctly.

    The clients themselves have a variety of support needs and come from a variety of backgrounds including low level substance misuse issues, ex-rough sleepers and those who are battling various low - medium risk mental health issues.

    Role responsibilities:

    • Delivering initial assessments with clients who are newly referred to TST South, and 3 months initial resettlement support if clients move into a Property
    • Ensure clients are set up with the correct welfare and benefits
    • Set up rental transactions between the client and the landlord
    • Assist the client with settling into their accommodation
    • Work collaboratively with the people we support to create and review Risk Assessments and Support Plans in line with TST service timescales
    • Work in partnership with TST colleagues, other services, including health, housing, social care, and Employment & Skills teams, to deliver holistic, coordinated support to clients
    • Contribute towards service evaluation and improvement through engagement with the people we support, with colleagues, and with other key stakeholder
