Job description

Support Worker - Streatham Supported housing for Young People

Pay: £13.16 holiday pay = £14.74 per hour

Working Pattern: Mon - Fri (Early and late, 9-5 or 12-8)

One weekend a month

Duration: 3 months with potential Temporary to permanent offer

Location: Streatham

We are looking for a Support Worker to provide housing related support to young people leaving care and moving into supported accommodation, together with providing housing and property management with the aim of achieving a successful move-on and other measurable outcomes including supporting service users to achieve independence.

Duties and responsibilities include but are not limited to:

  • Engage with aftercare leaving team, social services and local authority housing services, in order to generate referrals for the service as required
  • Plan and deliver an effective support and safety plan with the service user referring to specialist agencies as required. The progressions/outcomes identified, delivered and service user contact should be recorded, monitored, reviewed, and updated regularly and in line with contract requirements
  • Maximise rent collection, ensuring successful housing and other welfare benefit claims are submitted and maintained, taking proactive action to minimise rent arrears in line with targets, monitoring the account and recording all actions and where necessary taking appropriate action when arrears arise in line with policy and procedure

21 clients in total.

Do they oversee a caseload?

5 Clients. Weekly support meetings.

What young people are like in terms of level of need?

Low to medium. 16 - 21, children of care.

What purpose of service is like?

Referred to being social services, get them prepared for independent living, and help them support.
