Move on Coordinator
I am currently looking for a Move on Coordinator to work with one of our charities based in Westminster. The role will be working Monday - Friday 8.00am - 4.00pm and 10.00am - 6.00pm which will be on a weekly rotation.
Responsibilities within the role:
- Managing the referrals coming into the service and ensuring assessment are allocated within the team
- Ensuring move on plans are built into clients plans via a multiple disadvantage lens
- Holding a case load of clients currently living within the step down service
- Respond in a psychologically informed way to challenges posed by clients with Complex Needs
- Conduct skilled and sensitive assessments with people who may be ambivalent about taking up support
- Working with residents to maximise benefits
For the Move on Coorindator role you will need an Enhanced DBS within the last year or one that is on the update service.
