Skills
Strong communication skills are required as the post holder will be providing care to service users who may be mentally unwell, have experienced trauma and who have struggled to engage with mainstream services.
Applicants will be able to work flexibly and with partner agencies, in order to give service users the best possible care.
The Role
The post holder will be a dynamic, driven individual who is committed to providing assertive outreach in order to engage or re- engage the homeless population into well-being services and drug and alcohol treatment. They will be able to work independently, but also part of a supportive team:
- To carry out triage and comprehensive substance misuse assessments, risk assessments and develop person-centred recovery care plans
- Conduct street outreach to engage/reengage homeless individuals using substances
- Deliver satellite provisions within local services e.g. GPs, probation, hostels, etc.
- Co-facilitate therapeutic groups
- Deliver harm reduction advice
- Work in collaboration with partner agencies in housing, immigration, healthcare, etc.
