Job description
I am currently looking for a candidate to work within one of our charity's hospitals based in Westminster. The role is a Senior Hospital Discharge Support Worker and the working hours are Monday - Friday 9.00am - 5.00pm. The post is for around 1 month currently, but it looks like there could be a possible extension.
The main purpose of the role is to work with residents who are returning to, moving into, independent accommodation.
Responsibilities
- Initial assessments of referrals to the Hospital
- Holding a caseload of clients to carry our Key working sessions resulting in SMART plans
- Risk & Needs assessments of your clients
- Dealing with safeguarding concerns
- Working alongside social workers and other health care professionals
For this role you will need an Enhanced DBS within the last year or one that is on the update service.
