Job description

We have an exciting opportunity at a Northampton Substance Misuse Recovery Service, this opportunity is for someone who is passionate about helping people and making a difference. This role will be based in and around the Northampton, willingness to travel where necessary via own transport or public transport.


We are looking for a Outreach Practitioner to manage a caseload of service users presenting with drug and alcohol issues. The role will involve offering a range of harm reduction interventions and structured key working, helping engage service users in a care-planned and recovery focused way to attain improved health outcomes and achieve their personalised treatment goals.

Responsibilities of a Outreach Practitioner

  • To engage young people in a variety of interventions aimed at increasing choice and control in their lives, using a young person centred, assertive outreach approach
  • Supporting young people from the point of entry into our service, through their intervention and after-care
  • Providing screening, co-produced assessment, psychosocial interventions, integrated and outcome focused care planning, after-care support, and onward transition to other services as required
  • Identifying and responding effectively to safeguarding concerns. This may involve supporting and educating family members, making safeguarding referrals to the local authority and participating in all relevant multi-agency meetings, including: Child Protection meetings; Complex Case Review panels, and Strat Meetings.

Requirements of a Outreach Practitioner

  • Proven track record working as Outreach Practitioner or simialr role
  • Managing a caseload of up to 50 at one time
  • DBS on the update service or issued within the last 12 months
  • Available to start ASAP / Has access around the Northampton area - Car driver ideal
