Outreach Worker

Up to £10.63 per hour

Job description

Morgan Hunt are looking to recruit an Outreach Worker to work with a housing provider covering the Preston area on a permanent basis.

As an Outreach Worker, you will manage your own caseload of young people and support them towards living independently and sustaining a tenancy.

The role - Outreach Worker

  • Providing ongoing assessments of service users needs
  • Conducting welcome and induction meeting with service users to establish their needs and to ensure the service user understands their tenancy agreement
  • Arranging appointments with various partner agencies and liaising with designated Social Worker around progress and individual needs of service users
  • Supporting service users to develop their independent daily living skills through things such as budgeting, healthy eating, healthy relationships and accessing health services
  • Accompanying service users to participate in activities of interest within the community
  • Preparation of reports and keeping accurate record around the progress of clients
  • Assisting with move on of service users when required

The candidate

As an Outreach Worker, you will have experience of providing floating support to those with complex needs, managing your own caseload and ensuring accurate records are maintained to monitor service user engagement.

Due to the role involving travel around Leeds and Bradford, you must be a car driver and have access to a car for work. You must hold a current Enhanced DBS dated within the last 12 months / on the update service or be prepared to get one.
