Our clients mission is to prevent and resolve homelessness, social exclusion and housing poverty by providing practical, realistic, tailored support so that each person can find a route out of homelessness, maintain a home and achieve their individual potential.
Our Liverpool City Region Assertive Outreach Service provides direct support to people sleeping rough or living a street lifestyle. The aim of the Service is to reduce rough sleeping; develop local referral routes into Housing First and other commissioned services; improve health and wellbeing and improve the pathways between partners such as the Criminal Justice system, mental health services and homelessness services.
To achieve these aims our client works in partnership with a number of stakeholders to provide an integrated package of support that addresses both health and social issues including crisis intervention, support and advice around access to accommodation, treatment, health and wider support and care services.
With a good working knowledge of Housing and Welfare Rights, the Assertive Outreach Worker will have at least 2 years' experience of working in a homelessness, housing or support setting, experience of developing personalised support plans and providing advice and support to people about housing, benefit maximisation, substance misuse problems, domestic abuse, care and mental health services.
Highly organised and with the confidence to work well in a multi-agency setting, you will be comfortable using your own initiative to make informed and effective decisions. As this is a 7 day service, we are looking for staff who have the ability to work within a flexible rota which will also cover some early mornings, late evenings and weekends by arrangement. Car use is essential as the City Region covers a large geographical area.
