Our client, a leading homelessness charity based in Hackney are looking for a Homeless Assessment and Resettlement Worker to work in a busy homeless assessment centre. Due to a company restructure a position has become available to support the charity in the assessment and resettlement of homeless clients with high support needs referred by street outreach teams.

Candidates for this role will need:

- Extensive housing knowledge and knowledge of the entitlement homeless clients have under different housing laws/ acts

- Experience and knowledge of the Homeless Reduction Act

- Experience of working with the homeless

- Experience of working in a homeless assessment capacity

- Resettlement experience and knowledge of housing options

DBS within 12 months of issue or on the online update service

Candidate duties:

- Case working a caseload of clients residing at the assessment centre

- Conducting needs and risk assessments and putting in place support plans

- Presenting clients with resettlement options

- Assisting clients to obtain their welfare, housing and related rights

- Referring clients on to relevant specialist support agencies for their needs

- Accurate recording of data onto CHAIN

- Liaising with external agencies on behalf of clients

Supporting people with severe mental illness, you will offer outreach and engagement to help those to access their annual comprehensive physical health check.

Based in the community of South-West London, this role is outreach based, meaning you will be working in various locations including clinics, GP practices and office bases.

Helping individuals to identify barriers and make reasonable adjustments to aid those to access their health check, this rewarding role is suitable for someone with experience within the mental health sector, having worked front line with those with complex mental health needs.

Our social care client is looking for a hospital discharge worker, supporting clients who suffer with mental health issues leave hospital and move into accommodation. This is a vitally important role due to the fact that you will be identifying those who are ready for move on, assessing their needs and working with external partners to assist with their transition into their accommodation.

Duties:

  • Identify clients ready for discharge in Lambeth hospital
  • Assessing clients needs and suitability to move on from the ward into a variety of accommodation settings including step down services, own home or supported accommodation in accordance with their needs
  • Clients: All mental health clients of various needs
  • You will be working within an NHS setting, at home and within the community
  • Complete professionalism, attention to detail, understanding of safeguarding, understanding of mental health and reliability are required in this role
  • Not a case load role, only clients that receive more attention are those who go to the step-down services to ensure they settle in
  • External services: social workers, home treatment teams, supported accommodation staff and NHS staff
  • home visits on occasion but no lone working

The service users are ex homeless with multiple support needs including mental health, drug and alcohol addiction and some physical health needs.

The successful candidate will be responsible for the project's performance against its contract, monitoring, and reporting to stakeholders in meeting the required targets.

Duties:

  • The service manager is responsible for a team within a geographical or functional area. You will report into the Area Manager and will be a member of the relevant Area Management Team

Our client

Morgan Hunt is working with a charity that provides housing and domestic abuse support to homeless men and women in Greater Manchester. Our client works with individuals to overcome experiences they have had an empower them to build up the skills and resources they need to take on a tenancy of their own and maintain it in the long term. They can make positive choices about their future due to information and support offered from the organisation.

Our client is looking to recruit a temporary Tenant Support Worker for approximately 2 months. This is a full-time post (36 hours per week) and will include a mixture of early shifts and late shifts. Every 4 weeks you will be required to work a Saturday and Sunday 8am - 8pm with days off in the week to accommodate.

The role is based at their female only accommodation project in Rochdale which contains individual flats and is staffed 24 hours a day. Floating Support is also offered to clients within the community for up to 6 months.

The role - Tenant Support Worker

As a Tenant Support Worker you will;

  • Provide a high-quality supported housing service to individuals accessing services.
  • Support clients on a one-to-one basis and assist them to make informed choices and remain safe.
  • Support clients to develop themselves and advance their independence, assisting them to access employment, education, and training opportunities.
  • Support in the delivery of high-quality group programmes.
  • Assess the needs of clients and work with them to devise a support plan to identify need.
  • Complete risk assessments which incorporate safety planning and that are reviewed on a regular basis.
  • Make referrals and work in conjunction with all relevant agencies in the best interests of clients.
  • Provide transitional resettlement support to clients moving into independent accommodation.

The candidate - Tenant Support Worker

  • An extensive understanding of homelessness and options available to people.
  • Knowledge of devising support plans, identifying risk and devising risk management plans to ensure outcomes are achieved.
  • Up to date knowledge of homelessness, housing, welfare benefits and legislation.
  • Knowledge and experiencing of responding to safeguarding concerns.
  • Liaising with other agencies and signposting to relevant services within the community.
  • Computer literate with a range of IT programmes.
  • To ability to manage a demanding workload and work in a fast-paced environment

Due to the nature of this role, a current Enhanced DBS is required dated within the last months or registered on the update service.
