Job Overview:

Assist street outreach workers in providing support to rough sleepers. Engage with clients, assess their needs, and help them access accommodation and services.



Key Responsibilities:

• Conduct outreach Monday-Friday
• Collaborate with agencies to provide housing and support options
• Advocate for clients' welfare and healthcare needs
• Manage a caseload, offering guidance until clients are housed
• Maintain accurate case notes on CHAIN and other databases
