This assignment has been deemed inside of IR35.

As a Housing Solutions Hospital In-Reach Officer, you will collaborate closely with hospital staff to identify individuals in need of housing solutions upon discharge.

Ideally, the successful candidate will have the ability to:

• Analyse information and make informed decisions based on individual circumstances.
• Showcase sensitivity and empathy in dealing with clients who may be experiencing health issues, homelessness, or other challenges.
• Use advocacy and negotiation skills to effectively represent the interests of clients and overcome potential barriers to securing suitable housing.
• Develop and implement individualised housing plans.
• Manage multiple cases and deadlines effectively using strong organisational skills.
• Use their knowledge of local housing resources, support programs, and community services.
• Deliver educational sessions and outreach activities to raise awareness about available housing solutions and support services.
• Prepare reports and documentation for internal and external use.
• Use their strong problem-solving skills to address challenges and find creative solutions for individuals facing housing insecurities.
• Conduct comprehensive assessments to understand the unique housing needs of individuals.

This role requires a valid Enhanced DBS Check.
