Outreach Team Leader - South West London

In order to successfully conduct Integrated Outreach services, the Outreach Team Leader oversees setting shift patterns and procedures that guarantee meaningful contact is established and maintained with every rough sleeper in the borough.

Together with this, they must provide full support to a team of outreach workers and rough sleeper navigator case workers, which guarantees that efficient assessments and referrals are made.

In addition, they must work with each client sleeping in the borough to develop action plans that clearly outline a way off the streets.


Responsibilities:

· Create performance goals and quality assurance procedures to support service delivery, and track team members' collaboration to make sure these are fulfilled.

· Assisting employees by establishing specific goals and providing frequent staff supervision.

· To accurately know how many, what kind, and what demographics of persons are sleeping on the streets in the borough, as well as where they are thought to be

· To keep a close eye on CHAIN/Inform records, making sure that every interaction and evaluation is documented in accordance with the CHAIN/Inform recording guidelines.

· To aggressively raise awareness of the Services and sustain positive working relationships with other borough businesses, community organisations, and service providers.

· Actively promote the service by using client case studies and best practice examples and manage and provide the service in accordance with contract requirements and subcontracting agreements.


Experience Required:

· Experience in the field of homelessness.

· Strong IT abilities with the capacity to quickly and accurately capture data on IT platforms like Inform and CHAIN.

· An understanding of data management and GDPR.

· Awareness of the services, pathways, and circumstances that are pertinent to the existing drug and alcohol offering.

· The capacity to function as a member of a management team while upholding productive working connections with coworkers, clients, the community, and other outside organisations.

· Understanding of the laws and resources available to homeless people in the areas of employment, immigration, housing, and health.

· Line management experience, encompassing hiring, onboarding, overseeing, and performance evaluation.
