We are currently recruiting for a Young Person Support Worker to join a dedicated service in Lewisham, supporting young people who have recently left the care system and are transitioning into independent living.

Key Responsibilities:


βœ… Manage and support a caseload of young people aged 16-25
βœ… Assist clients with developing independent living skills, including budgeting, cooking, tenancy management, and accessing education or employment
βœ… Complete support plans, risk assessments, and regular keywork sessions
βœ… Promote positive wellbeing, safeguarding, and personal development
βœ… Support young people to access benefits, healthcare, training, and community services
βœ… Maintain accurate case notes and ensure all records are up to date
βœ… Work in partnership with local authorities, social workers, and external agencies


Requirements:

βœ” Previous experience supporting young people, care leavers, or vulnerable adults
βœ” Knowledge of safeguarding and risk management procedures
βœ” Experience completing support plans and maintaining case records
βœ” Strong communication and relationship-building skills
βœ” Ability to work independently and manage a varied caseload
