We are currently recruiting for an experienced Outreach Worker to join a busy and supportive service based in Westminster. This is an excellent opportunity for someone with experience supporting vulnerable adults and rough sleepers, managing complex cases, and delivering community-based outreach support.
Working as part of a dedicated team, you will conduct daily outreach shifts, engage with individuals sleeping rough, and provide practical and person-centred support to help clients access accommodation and essential services.
Key Responsibilities
- Conduct daily outreach shifts across Westminster to identify and engage rough sleepers.
- Complete assessments and develop tailored support plans for clients with complex needs.
- Manage a caseload of vulnerable individuals, providing ongoing support and case management.
- Support clients to access accommodation, healthcare services, benefits, and specialist support agencies.
- Build positive relationships with clients to encourage engagement and progression.
- Work closely with local authorities, housing providers, healthcare professionals, and partner agencies.
- Maintain accurate records, case notes, support plans, and risk assessments.
- Respond effectively to safeguarding concerns and crisis situations.
- Support clients towards sustainable housing outcomes and increased independence.
- Attend team meetings and contribute to service development initiatives.
Essential Requirements
- Previous experience working within homelessness, rough sleeping, housing support, or outreach services.
- Experience managing complex and challenging cases.
- Strong understanding of the issues affecting vulnerable adults, including homelessness, mental health, substance misuse, and social exclusion.
- Ability to build rapport and engage individuals who may be reluctant to access support.
- Excellent communication, organisational, and report-writing skills.
- Experience working within a multi-agency environment.
- Competent IT skills and ability to maintain accurate records.
- Ability to work independently and manage a varied workload.
