Outreach Worker

We are recruiting an experienced Outreach Worker to support rough sleepers and vulnerable adults experiencing homelessness, helping them access essential services and secure suitable accommodation.

Location: Chelsea & Kensington
Contract: Temporary, Ongoing
Hours: Rolling rota, 7 days a week

Key Responsibilities:

• Engage and support rough sleepers and vulnerable adults.

• Complete needs and risk assessments and support plans.

• Assist with accommodation, healthcare, benefits and referrals.

• Work with local authorities and partner agencies.

• Maintain accurate case notes and follow safeguarding procedures.

• Promote independence and long-term housing solutions.

Requirements:

• Experience in homelessness, outreach or supported housing.

• Knowledge of safeguarding, risk assessments and complex needs.

• Strong communication and relationship-building skills.

• Ability to work independently and manage a caseload.
