
Floating Support Practitioner

About the Role:
As a Floating Support Practitioner in Solihull, Birmingham, you will deliver a range of services including advice, assessments, brief and structured interventions, group programmes, and training courses to service user homeless or at risk of homelessness.
This role operates across various settings such as primary care, arrest referral, hospitals, youth services, and outreach locations; wherever support is needed most. A car driver is not necessary but highly desired.
Key Responsibilities of a Floating Support Practitioner
- Provide advice, screening, and both brief and comprehensive assessments for individuals, couples, and families
- Deliver brief and longer-term structured interventions, based on recovery-focused approaches such as CBT and the model of change
- Develop, implement, and review personalised care plans, monitoring progress and outcomes
- Offer case management and ensure integrated care pathways
- Support family members and others affected by substance use
- Facilitate therapeutic group programmes and peer support groups
- Maintain accurate and up-to-date records and data
- Work in partnership with a wide range of professionals, agencies, and community groups
- Promote services at public events and within the community
- Collaborate effectively as part of a supportive multidisciplinary team
- Provide supervision and support to volunteers and peer mentors
- Contribute to the development of family interventions and offer case-based consultation
- Ensure compliance with all safeguarding procedures and statutory responsibilities
About You:
We're seeking candidates who:
- Have experience within Homelessness/Housing or in a similar Floating Support Practitioner s role
- Value diversity and inclusivity in their work
- Understand substance misuse within the framework of recovery
- Are familiar with CBT and other evidence-based interventions
- Are excellent communicators with strong organisational skills
- Have experience working collaboratively in a multi-agency environment
- Can work flexibly across different settings
