About the Role

We are looking for a committed and reliable Outreach Worker to provide high-quality, person-centred support to vulnerable adults within supported accommodation services.

This is a flexible, temporary opportunity suited to someone with experience supporting adults with learning disabilities, mental health issues and a history with substance misuse. The role focuses on promoting independence, safeguarding wellbeing, and helping individuals achieve positive outcomes.

Key Responsibilities

As an Outreach Worker, you will:

• Provide practical and emotional support to adults at risk
• Build positive, trusting, and professional relationships
• Support individuals to develop independence and life skills, including budgeting, cooking, housing readiness, household management, and using public transport
• Help individuals access education, training, employment, health services, community resources, and wider support
• Advocate for individuals and ensure their views are heard
• Contribute to support plans, risk assessments, statutory reviews, and reports
• Keep accurate records of support, progress, incidents, and outcomes
• Identify, record, and escalate safeguarding concerns appropriately
• Liaise with social workers, personal advisers, housing teams, police, health services, and other agencies
• Complete property health and safety checks with supported individuals
• Attend team meetings, supervision, appraisals, and relevant training

Essential Requirements

Applicants should have:

• Experience of working, volunteering, or relevant life experience with children, young people, adults at risk, or families
• Understanding of safeguarding
• Ability to build and maintain professional relationships
• Strong communication and interpersonal skills
• Good organisational and administration skills
• Ability to manage a caseload and prioritise tasks
• Ability to keep clear records and produce reports
• Basic IT skills, including Microsoft Office
• GCSE/O-Level English, or equivalent
• A relevant Level 3 qualification in Health and Social Care, or equivalent experience, or willingness to work towards this

Desirable

• One year's post-qualifying experience supporting young people aged 16+ or adults at risk
• Knowledge of trauma-informed practice
• Understanding of housing and benefits systems
• De-escalation skills
• Full clean driving licence with business insurance
• Willingness to undertake continuous professional development
